Studies show that approximately 14% of the 3.5 million U.S. truck drivers are living with diabetes—about 50% higher than the general adult population. Given the safety-sensitive nature of commercial driving, proper medical evaluation and ongoing management are critical.
Under FMCSA regulation 49 CFR §391.41, a driver is not physically qualified to operate a commercial motor vehicle if they have insulin-treated diabetes—unless they meet the requirements outlined in §391.46.
The September 19, 2018 FMCSA final rule (§391.46) modernized this standard. It allows Certified Medical Examiners (CMEs), in collaboration with the driver’s treating clinician, to determine whether an insulin-treated individual can be certified. This rule replaced the previous federal exemption program, streamlining the process for drivers and employers.
Key Requirements for Certification:
- Drivers treated with insulin must provide the Insulin-Treated Diabetes Mellitus Assessment Form (MCSA-5870)
- The form must be completed, signed, and dated by the treating clinician within 45 days of the DOT physical exam
- The CME uses this information, along with the exam, to determine certification eligibility
What to Expect During the DOT Physical:
- A urine sample will be collected to assess glucose levels
- Drivers with diabetes or pre-diabetes should bring their most recent Hemoglobin A1C (HbA1c) results
- Many examiners look for A1C levels between 7–10% as an indicator of controlled diabetes
